PRACTICE

Real cases that show how it works

Below are de-identified cases from our centers’ practice: the site, the method, the dose and what follow-up showed. Patient personal data is never published.

METHOD

What is known about radiosurgery overall

The figures below come from international literature on radiosurgery as a method itself — not from the results of our centers

92%

five-year survival in early lung cancer

90%

local control for spine tumors

97%

five-year progression-free survival in prostate cancer

8 600+

publications on radiosurgery in the international literature

CLINICAL EXAMPLES

Six cases from our centers’ practice

De-identified: only the site, method, dose and follow-up results are kept

01

Lung cancer with atelectasis: the plan was rebuilt during treatment

Patient
Male, 64 years old
Diagnosis
Squamous cell carcinoma of the right lung complicated by atelectasis
Method
Stereotactic radiotherapy with the target outlined on co-registered images
What we did

Initial regimen — PGTV 6 Gy × 3 fractions. After three fractions the right lung re-expanded and the plan was rebuilt: PGTV 4 Gy × 12 fractions.

Follow-up result

The atelectasis resolved during radiotherapy; the rest of the treatment proceeded on the adjusted plan — without interruption and without changing the method.

02

Small liver cancer: the patient declined surgery

Patient
Male, 61, hepatitis B carrier
Diagnosis
A nodule in segment S5 measuring 2,03 × 1,63 cm; biopsy — well-differentiated hepatocellular carcinoma, stage IA
Method
CyberKnife to the liver lesion, 12 Gy × 5 fractions
What we did

The patient declined resection. Based on multimodal imaging, the lesion was irradiated stereotactically — no puncture and no anesthesia.

Follow-up result

A partial response was still present at 7 months; one year after radiotherapy no local recurrence was detected.

03

Pancreatic cancer with liver metastasis: three years of follow-up

Patient
Male, 59
Diagnosis
Pancreatic cancer with liver metastasis, plus an untreated thyroid cancer
Method
Palliative radiotherapy of the pancreatic lesion and chemotherapy, then precision radiotherapy of the single liver metastasis
What we did

Surgery was contraindicated because the tumor had grown into the vessels. Palliative radiotherapy and 6 courses of gemcitabine with nab-paclitaxel were given; later — radiotherapy of the single metastasis in segment S1 of the liver over 5 fractions.

Follow-up result

On imaging the pancreatic tumor is in durable remission; follow-up PET-CT showed no recurrence or metastasis. Overall survival — 3 years; during that time a radical operation for thyroid cancer was performed.

04

Giant liver tumor with portal vein thrombosis

Patient
Poorly performance-status patient, ECOG PS 2; comorbidities — hypertension and recurrent cholecystitis
Diagnosis
A 13,2 × 11,6 cm tumor of the right liver lobe with multiple intrahepatic metastases; thrombosis of the branches and main trunk of the portal vein; stage CNLC IIIa
Method
Radiotherapy 40 Gy over 5 fractions to the tumor thrombus, then SCART to the primary lesion: PTV 15 Gy / 3 fractions, STV 63 Gy / 3 fractions, combined with targeted and immunotherapy
What we did

Surgery was not possible. The tumor thrombus was irradiated first to preserve the remaining liver function, then SCART was used — a high dose to the core of the tumor with a reduced dose at the periphery.

Follow-up result

The tumor shrank by about 90%, the tumor thrombus almost disappeared and the abdominal bloating resolved. The AFP tumor marker returned to normal, compensatory hypertrophy of the left lobe developed and liver function improved.

05

Prostate cancer with a solitary metastasis in the L4 vertebra

Patient
Male; oligometastatic hormone-sensitive prostate cancer
Diagnosis
cT2bN0M1b, stage IVB; PSA 180,153 ng/ml, Gleason score 4+4=8
Method
Long-term (2–3 years) androgen deprivation + SBRT to the prostate and local irradiation of the metastasis in L4
What we did

The L4 metastasis was irradiated on a linear accelerator (GTV 8 Gy × 5, CTV 6 Gy × 5). The primary tumor was treated on the CyberKnife, 7,25 Gy × 5, with the dose-painting technique and tracking by fiducial markers.

Follow-up result

The pain in the right lower limb resolved completely and the PSA level dropped quickly.

06

Arteriovenous malformation of the brain

Patient
Intracranial arteriovenous malformation complicated by hemorrhage
Diagnosis
AVM with persistent blood flow in the nidus after three endovascular interventions
Method
CyberKnife, 8 Gy × 3 fractions
What we did

Three endovascular embolizations were performed over a year and a half, but the nidus could not be closed completely and the hemorrhages recurred. Stereotactic radiotherapy was delivered.

Follow-up result

Six months later MRI and MR angiography showed a marked reduction of the nidus: most of the pathological vessels were closed and no new hemorrhage was detected.

How to read these cases

The response to treatment is individual: a similar diagnosis does not mean the same result. Decisions on method and dose are made by the board based on your documents and examination data — before treatment starts.
